About Palliative Care

  • Treatment focuses on a chronic medical condition
  • Managed by a physician group and covered under traditional insurance
  • Most services are subject to a deductible and payable at 80%
  • Care is provided wherever the patient calls home – be that a private residence, Assisted Living Residence or Skilled Nursing Facility
  • Addresses the patient’s physical, mental, social, and spiritual well-being, is appropriate for patients in all disease stages, and accompanies the patient from diagnosis to cure.


About Hospice Care

  • Available to terminally ill patients with a prognosis of 6 months
  • Treatment focuses on symptom control and comfort
  • Care related to the hospice diagnosis is covered 100% by Medicare, Medicaid and most commercial insurances, including equipment and medications
  • Care is provided wherever the patient calls home – be that a private residence, Assisted Living Residence or Skilled Nursing Facility
  • Makes the patient comfortable and prepares the patient and the patient’s family for end-of-life when it is determined treatment for the illness will no longer be pursued
